This one almost leaves you speachless. With my simple mind I cannot come up with the words to adequately describe how absurd this latest re-issue of a March 2003 warning really strikes at the heart of how Medicare seems to be trying really hard to make it impossible to provide product at a reasonable reimbursement even when you have a written order in hand from a prescribing doctor. As a member of the bottom of the food chain (DME), it is clear that the physicians can basicaally write their own ticket (printing money so to speak) and we are at the mercy of the physicians to sign paperwork that we must have prior to billing, and that they do not want to sign nor to sign up for PECOS for which they are not accountable. If they do not get signed up in time, they are not penalized, the patient and the DME dealer are the ones paying the price. We are also forced to somehow make the patient go in for face to face office visits,in a timely manner, or we cannot bill and we cannot pick up the equipment. At some point all DME dealers are going to say enough is enough and leave it to the brilliant minds at CMS to solve the MAJOR PROBLEMS that they are creating as we speak and you thing spending is out of control now, just you wait till you have everything running through part A.

One upside to this is, that it has provided a high level, mutually important topic for us to discuss with physicians in our community and those personnel who support them. Physician offices that ordinarily would not "see" us from the DME perspective are welcoming us in to provide this info/education and in the course of doing so, we have the opportunity to promote what we do and reinforce our image as credible, informed, and helpful (to them). I am determined to find the positive in our challenges and in this one, this is it. Thanks for sharing what you know with the DME community. PK
